Tehama Oaks utilizes both a Transition Specialist and a Parent Liaison to support student transitions.
 
When a student is enrolled with Tehama Oaks for 20+ consecutive school days a full transition meeting takes place to identify supports and barriers for student success, and outlines a plan for the student to reach graduation and college/career goals.

Transition Upon Enrolment
 
  • Immediate Enrollment: Students are enrolled in school as soon as they enter the facility.
  • Academic Testing: Within the first week, students are tested in math and reading to identify strengths and areas needing extra help.
  • Therapist Meeting: Students meet with our in-house therapist to address social-emotional needs.
  • Resume Workshop: Each student participates in a workshop to develop job-seeking skills and create a resume.
  • Career Interests: Students identify their career interests to help guide future educational and vocational planning.
 
Transition After Release
 
  • Transition Meeting: A meeting is held with the student, parent, receiving school, and probation to ensure a smooth transition.
    • Identify Barriers: Discuss any potential barriers and create a plan to overcome them.
    • Graduation Plan: Establish the student’s graduation plan and any further educational requirements.
    • Programs: Review any additional programs the student must complete after leaving.
  • Transcript Updates: Transcripts are updated as soon as they are released to ensure that all academic records are current.
  • School Handoff: A complete handoff to the new school is done, ensuring the receiving school is prepared to support the student.
